Long Term Care Program
The Long Term Care Program (LTC) is funded by the province through the Ministry of Health. It provides information, advocacy and practical support that allows individuals to remain in their homes for as long as possible. It also helps to ease the weight of care provided by informal caregivers such as extended family. LTC services are available to self-identified Métis persons of all ages, and their extended families. The emphasis is on service for elders at risk and those trying to overcome barriers like low income, geographic isolation, language or disability in accessing care.
LTC services are coordinated with other community based programs and may include referrals to other agencies for Meals on Wheels, Adult Day Programs, Nursing, and Home care.
The following support services help to ensure people continue to live a full and independent life in their home community:
Transportation
Volunteer drivers work with community based LTC workers in communities with large Métis populations to transport people to medical appointments. Both private vehicles and public transportation are used and demand for this service continues to grow.
Social & Safety Visits
Regular visits and phone calls, based on client wishes, help to provide support for isolated or ill clientele while ensuring their health and safety. Regular contact is made through visits by MNO staff or volunteers is arranged by LTC Coordinators.
Community Support Services
The LTC Coordinator is knowledgeable about all the services available to Aboriginal people in their area and provides information, referrals, advocacy and access to mainstream or other long-term care services such as Community Care Access Centres.
Each LTC site is wheel chair accessible and smoke free. Community Coordinators work with Community Councils to fund raise so that additional services not funded by the Ministry of Health under the program can be provided. The valuable role of volunteers in the delivery of the LTC program cannot be overstated because they ensure that many who would not receive support are served. As a result of the Long Term Care Program many Métis citizens, who might not otherwise be able to remain in their homes are able to do so.
